Health / Social well being assessment
The Health/Social well being assessment measures your ability to manage in your current accommodation. The Housing Executive uses this assessment to decide which applicants will benefit the most from being rehoused. The Housing Executive can award points in four categories: functionality, support needs, social needs, and complex needs.
Functionality
Functionality points are given after the Housing Executive assesses your mobility inside and outside your home. If you or anyone who will be moving with you have any mobility problems that impact on your ability to access rooms or facilities in your current accommodation you may get points. More
Support/Care needs
You can be awarded additional points if you have support or care needs. Although the Housing Executive assesses every applicant, you will only get the points if you have applied for supported or sheltered accommodation. More 
Social needs
The Housing Executive gives you points if you are experiencing social problems because of your housing circumstances. The Housing Executive awards points for social needs in two categories: ‘Primary social needs’ and ‘Other social needs’. More
Complex needs
If you need extra support as well as new accommodation, you may be a ‘Complex needs applicant’. You can apply for specialist supported housing or general needs housing with a care package. More
