The application form asks questions about your personal details, your housing situation, your income and the type of work you need. You need to get a referral from an Occupational Therapist when applying for a Disabled Facilities Grant.
You may be able to get help from Shelter NI or Fold to complete your application. You can't get a grant if you rent from the Housing Executive or a housing association.
Your personal details
You must tell the Housing Executive about yourself and anyone else who will live in the house with you. For example, your partner. You must tell the Housing Executive if anyone who will live in the house is over 60 years of age.

Your housing situation
You must tell the Housing Executive whether you:
- are a landlord
- are a private tenant
- own your home.
If you are a landlord or own your home, you must be able to prove that you have at least five years left on the lease.
If you are a tenant you must be able to prove to the Housing Executive that you are responsible for carrying out the repairs. For example, your repairing obligations are laid out in your tenancy agreement.

Your income
You must tell the Housing Executive about your income. For example:
- your wages
- your benefits
- your tax credits
- do you have any stocks, shares or bonds.
You must also tell the Housing Executive if you own any property other than the grant property.

Type of work
The Housing Executive won't accept your application unless you describe the type of work that you want to carry out. You must also tell the Housing Executive:
- what type of property you need the grant for
- if the work is needed to help a person with a disability
- if you are looking for a grant to improve a house in multiple occupation (HMO).
Your application must include an estimate from a Housing Executive approved contractor. Contact your local Grants Office for a list of approved contractors in your area.

After completing the form
Return the completed form to your local Grants Office. The Housing Executive usually won't pay a grant if you start building work before you get formal written approval. Planning or building control is not the same as formal approval by the Housing Executive.
Don't start work if you unsure whether you have the correct permission. Contact your local Grants Office to make sure.

Can I get help to complete the form?
You can get help from your local Housing Executive Grants Office at any stage of your application. If you are elderly or have special needs you may be able to get help from external agencies like Shelter Northern Ireland or Fold Housing Association.
